WE NEED YOU...TO HELP US DEVELOP AND DELIVER THE BEST COMMUNITY DIABETES SERVICE IN THE WORLD

Within the Spirit family of companies we recently took over the running of a group of GP Practices in Leicester and we are looking to do things differently.  We care for over 1,200 people with diabetes and this number is increasing rapidly.


The Challenges as we see them:

  • Diabetes is a growing challenge and all practices are under pressure
  • Sub optimal blood glucose control is common within practice populations and this puts additional pressure on NHS resources
  • Compliance within care pathways and treatment plans is often problematic, in fact some people simply don’t turn up for review 
  • At the moment we have very limited access to diabetes experts within our practices. This leads to variation in the patient pathway.
  • The experience to start Insulin and GLP1’s is rare in primary care 
  • Many people with pre-diabetes are not being identified and are therefore going on to develop full diabetes
  • Many people with existing diabetes have not benefitted from structured diabetes education, at the moment education funding prioritises newly diagnosed patients.
  • Hospital services are busy and should be reserved for people needing specialist care, so they can be accessed as quickly and frequently as required.

Our Plan
We will develop a service for primary care based on best practice – making the world for people living with diabetes a healthier and happier place.
  • We plan a full audit of diabetes care in our practices to better understand the current pathway and their performance
  • We will bring together a new Primary Care MDT across our practices consisting of GP’s, DSN’s and specialist pharmacists. 
  • We will develop our award-winning diabetes education programs so that a wider group of people with diabetes can benefit
  • Our MDT will receive the best training – to become experts in diabetes and our pathways
  • We will personalise care so patients receive reviews as required, not just annually
  • We will support patients in achieving their blood glucose targets so they move along a defined treatment pathway.
  • It is not just about blood glucose – we will also improve holistic care for our patients
  • We will integrate technology to support our MDT and patients in achieving their goals
